Enrollment Form

Form is intended for people paying by credit card or 3rd party sponsors.

Enrollment Form

Subscriptions are for individual use only and may not be transferred. All subscriptions must be prepaid. You will be notified by email of access information when the enrollment form has been processed. All forms are processed with-in 1 week upon receipt of completed Enrollment Form and payment (unless during a holiday week).

Paying by Credit Card

  2. Create an account or log-in as a guest
  3. Select Volumes/Lesson to purchase
  4. Fill out Enrollment Form information and submit order and payment.

All Enrollments are completed manually.  You will not see a charge on your card until access is given and your purchase has been processed.  Every effort is made to process orders with-in 2 days of purchase however, it could take up to 1 week.

Paying by Check

  1. Download the enrollment form above
  2. Fill out the form
  3. Mail it along with your check to the address indicated on the form

Third Party Sponsors

If a third party sponsors your subscription, the Enrollment Form needs to be routed through them. You may email or fax the Enrollment Form to the appropriate person. They will forward the completed enrollment form to us once approved.
