Apply for Continuing Pharmacy Education

Do you have a program for which you would like to offer Continuing Pharmacy Education credits?  We can help!  We have extensive experience, competitive prices and a quick turnaround.  To begin, just fill out the Accreditation Application and review the checklists below. 

Choose the form that best fits your program.  Submit your application as soon as you have a program date and agenda.  We will not accept an application with-in 60 days for Multiple session events and 30 days for single session events, unless under special circumstances a $250 late fee will be applied.  After acceptance of the program, all required content must be submitted to the CPE office no less than 2 weeks before the event date [presentation materials and final agenda are excluded to this timeline].  Send all applications and questions to the CPE Administrator Christina Munoz at: cmunoz@salud.unm.edu

BASIC INSTRUCTIONS:

  1. Download and Fill out Application.
  2. Download Disclosure Statement form.  Have ALL speakers and event coordinators fill out and Sign Disclosure Statements. Include these with the application.
  3. Obtain current CVs for ALL speakers.  Include with application
  4. Download the Advertisement Checklist and review.  Include a copy of any draft advertisements already created.
  5. If an agenda/brochure or any other supporting documents have already been created, please include these with your submission for review.
  6. Send all of the documents above to Christina Munoz at cmunoz@salud.unm.edu [If any documents are missing and/or you do not have them yet please include a note in the email to indicate when this will be completed.  If you are unsure about certain fields or would like us to review content and/or help fill out a form we  would  be happy to do so.

You will be contacted by email with in 2-3 business days after application submission with additional information and/or questions on your event before approval.  Please note that an application needs to be submitted no less than 60 days prior to the event date along with ALL supporting documents [signed disclosures, CVs, advertisements/brochure/agenda and program content] 30 days before.  Required documents are required no less than 2 weeks prior to the event date.

WHAT TO EXPECT AFTER PROGRAM APPROVAL

  1. UAN(s) will be created.  An ADF will be emailed to you to include UAN details
  2. Advertisment template with UAN and submission requirements will be provided
  3. Instructions to distribute to participants during/after the event will be provided
  4. Access code(s) to event evaluations will be provided
  5. 30-40 days after the event
    • Invoice will be submitted (if applicable)
    • Participant and Evaluation Summary will be provided to program coordinator

Single session = 1 event/session occurring one time and/or can be given multiple dates
Session = block of time dedicated to 1 topic
Multiple session = several sessions during 1 event (conference/workshop etc.)
